Phone account / sub account

I can log into my account. The only phone in my account says it belongs to my wife and she has her own account. However, she has no account. If she tries to log in using her email address and use “forgot password” it says she has no account. How do I make this account into a single-owner account? Note: Remove account doesn’t work because it says she has no account.

Does your credit card get charged once or twice per month? Even if on the same day, do you get one charge for the total of the two phones or two charges?
